Abstract

Medication nonadherence remains a persistent barrier to therapeutic effectiveness, leading to avoidable morbidity, readmissions, and waste across healthcare systems. Smartphone-enabled pill dispensers (S-PDs)—electronic adherence aids that pair automated dosing cues with app-based reminders, real-time logs, and bidirectional alerts—have emerged as a promising solution. This manuscript examines the dual impact of SPDs on (a) patient compliance and (b) pharmacy workflow, with attention to human factors, data governance, and sustainability of adoption. We propose and detail a mixed-methods evaluation framework combining a quasi-experimental effectiveness study with a time-motion workflow assessment and qualitative inquiry. The methodology includes pre/post measurement of adherence (proportion of days covered [PDC], medication possession ratio [MPR], missed-dose rate) and service metrics (dispensing cycle time, call-backs, intervention documentation, and alert resolution time). A pragmatic study protocol is presented for community and hospital outpatient settings, including eligibility, onboarding, device calibration, alert thresholds, escalation ladders, and privacy safeguards. Hypothetical but realistic results drawn from a sample of adult patients with chronic polypharmacy (n=420) illustrate an improvement in mean PDC from 0.74 to 0.86,a 29% reduction in missed doses, and a 22% reduction in unscheduled pharmacist call-backs after three months. Workflow analysis suggests a 14% increase in initial setup time per patient offset by a 19% decrease in ongoing exception management after stabilization, yielding net neutral labor by week six and improved documentation completeness. Qualitative themes include increased patient confidence, occasional alert fatigue, and the importance of customization (snooze windows, travel mode, low-vision interfaces). We conclude that S-PDs can meaningfully improve adherence and rationalize pharmacy activity when implemented with clear protocols, inclusive design, and data-driven governance. Future work should examine cost-effectiveness, long-term persistence, and integration with e-prescribing, payer incentives, and pharmacist-led care plans.
